Daily At-Home Symptom Screening
This screening tool is designed to help parents, students, and staff determine if they should participate in in-person instruction at school or remain isolated at home. It should be used each morning before travelling to school for the day.

Parents & Guardians: Please use this screening tool to check your child’s symptoms each day before travelling to school, including getting on the school bus. If you determine your child should not attend in-person classes, please be sure to follow the guidelines on our COVID-19 Procedures page. If your child passes the screening, you are not required to submit the completed form to the school.

Faculty & Staff: Please complete a self-screening each day you will work from the building. If you do not pass the self-screening, please follow our established COVID-19 staffing procedures.

Daily Sympton Screening Tool
Complete this symptom screening ecery day you will participate in activities at the school building, and use the results to determine if you should attend or remain at home.
Section 1: Check for Symptoms
Take Your/Your Child's Temperature

Screen for any of the symptoms blisted blow:
 
Group A
One or more symptoms
Fever (100.4 or higher)
Cough- New uncontrolled that causes difficulty breathing (for students with chronic allergies or asthmatic cough, a change in their cough from baseline)
Shortness of breath
Difficulty breathing
Group B
Two or more symptoms
Sore throat
Runny nose/congestion
Chills
New lack of smell or taste
Muscle pain
Nausea or Vomiting
Headache
Diarrhea
Section 2: Close Contact & Potential Exposure
Have you had close contact with a person with confirmed COVID-19?  
Close contact is defined as being within six feet of an infected person for at leat 15 minutes.

When to Stay Home
Stay home and notify AACMS if you/your child meet any of the following criteria:
  • Have one or more symptoms in Group A.
  • Have two or more symptoms in Group B.
  • Are taking fever reducing medication.
  • Had close contact with (within 6 feet for 15+ minutes) or a household member has confirmed COVID-19.  (You must quarantine for 10 days AFTER the person in the household is released from isolation.)
  • If you are under quarantine or awaiting a negative COVID-19 test result due to travel, per the Governor’s November 17, 2020, Order: "Travelers entering Pennsylvania from other countries and states, as well as Pennsylvanians who are returning home from other countries or states must have a negative COVID-19 test within 72 hours prior to entering the Commonwealth or quarantine for 10 days upon entry into Pennsylvania."
